Miličić Dent applies the All-on-4 and All-on-6 concepts in completely edentulous patients or those patients who have lost almost all of their teeth and whose remaining teeth cannot be used as support for a prosthetic restoration – dental bridge.

What is All-on-6?

 

All-on-6 is an implant-prosthetic method carried out in two phases:

First phase: Placement of 6 premium dental implants in one jaw, on the preplanned positions, aiming to achieveprimary stability of the implants. Thanks to immediate implant loading, we place and fix a temporary bridge on screws within 24 hours.

Second phase: Fabrication of a permanent bridge, made of a more aesthetically pleasing and stronger material, which requires a few more days.

What is the difference between All-on-4 and All-on-6 implants?

 

  1. Number of implants

The names of the concepts indicate the number of implants being placed. In the All-on-4 concept, 4 implants are installed, while in the All-on-6 concept, 6 implants are used.

  1. Implant positions

In the All-on-4 concept 2 implants are placed laterally and 2 implants are placed vertically in the anterior part of the jaw. In the All-on-6 concept 4 implants are vertically placed in the posterior regions and 2 more implants are placed in the anterior part of the jaw.

 

  1. Stability

With the additional two implants, compared to All-on-4, the All-on-6 concept provides better stability for the bridge structure and optimal redistribution of bite forces.

Who is a candidate for All-on-4, and should choose All-on-6?

 

The All-on-4 concept is used for all patients whose bone structures allow the placement of a smaller number of implants specifically only 4 implants, to ensure a permanent and functional fixed-prosthetic therapy. For these patients All-on-4 is the optimal solution as 4 implants are sufficient to secure the stability of the prostheses. However, certain prerequisites must be met for the therapy in question. For instance, the condition of the bone structure and sufficient at the planned implantation position, the patient’s constitution, i.e. the assessed bite force of each patient, and several other parameters must be taken into account to ensure the success of the therapy.

The All-on-6 concept is recommended for those patients for who, after examination, the placement of a smaller number of implants deemed sub-optimal. In such patients it is better to install 6 implants, according to situation in the mouth cavity i.e. the state of the bone. The All-on-6 concept is more frequently applied in the upper jaw compared to the lower jaw due to the lower bone density in the upper jaw. Hence obtaining primary stability of implants in the upper jaw bone is more challenging, so it is often necessary to place a larger number of implants in order to enable immediate loading of the implants with the placement of a temporary bridge. More implants, i.e. support for the prosthetic work in this case, provide greater therapy security for the patient.

FIRST PHASE

EXAMINATION AND DIAGNOSTICS

In order to assess whether you are a candidate for this procedure, it is necessary to conduct diagnostics – examination of the oral cavity by an oral surgery specialist and a CBCT scan. CBCT gives us a 3D view that enables us to assess the bone condition (width and density) in the third dimension. This scan is crucial in determining whether we can place an implant in the bone at the intended position.

 

  1. Examination of the oral cavity by an oral surgery specialist.
  2. CBCT imaging and scan analysis.
  3. Photography of the patient’s current smile.
  4. Digital Smile Design – digital scanning and virtual presentation of the future smile to the patient.
  5. Planning of the oral surgery procedure using specialized software which measures the bone and positions the implants.
  6. Presentation of the planned treatment to the patient.

OPERATION

The procedure is performed under local anesthesia with the possibility of additional conscious sedation, should the patient choose so.

  1. Oral surgery specialist extracts the remaining teeth and removes inflammatory tissues, cysts, etc., and then levels the ridge to create an ideal foundation for implant placement.
  2. Six premium dental implants are installed using special techniques in order to achieve the primary stability of the implants. FIXATION OF THE BRIDGE AT THE DENTAL PRACTICE

We position and fixate onto the screw the bridge, which fully meets all functional and aesthetic requirements.

SECOND PHASE

The second phase follows after osseointegration, i.e., the process of firm integration of the implants into the surrounding bone, which takes 4-6 months. During this time, the patient uses the temporary bridge as if they were had their natural teeth.

Individual zirconia or all-ceramic crowns are placed on the framework, depending on the patient’s preference. They give artificial teeth a natural appearance and provide superior aesthetics compared to the temporary bridge.

 

What is the difference between a temporary and a permanent bridge?

 

A temporary bridge is made of a simpler material – acrylic, while a permanent bridge (also called definitive or fixed bridge) is made of more durable and aesthetically superior materials, which require a few more days for fabrication:

The fixed bridge consists of a full set of teeth, i.e. it consists of 12-14 crowns, while the temporary bridge consists of 10 dental facets.

Is All-on-6 a painful experience?

 

As already mentioned, All-on-6 is a minimally invasive method since the oral surgical/implant phase is performed under local anesthesia and/or conscious sedation. Conscious sedation is recommended for patients who have significant fear of dentists and of surgical procedures.

 

After the surgical procedure, the patient is provided with:

  • Antibiotics to prevent bacterial penetration and inflammation around the implants during the early stage of the therapy.
  • Pain relief tablets.
  • Cold compresses.
  • Dietary instructions.
  • Oral hygiene instructions.
